Florida Divorce Without Dependent Children or Property Client Intake

Based on the attached Florida Supreme Court Approved Family Law Form 12.901(b)(3), Petition for Dissolution of Marriage with No Dependent or Minor Child(ren) or Property, revision 02/18. This intake collects the proposed petitioner’s facts, document status and instructions for review before document preparation. This questionnaire gathers facts for document preparation and identifies responses needing follow-up.

Please complete this questionnaire so Mirchi26 can review your request for document preparation for a Florida simplified dissolution of marriage. Answer accurately. Choose “Not sure” when you do not know an answer. Do not guess or sign for your spouse.

We provide document preparation services only. We do not provide legal advice, choose legal remedies for you, represent you in court or guarantee court approval. This questionnaire is not a court filing, a signed petition or a marital settlement agreement. You may consult a Florida attorney about your rights before proceeding.

Do not enter Social Security numbers, full account numbers, card details, passwords or identification numbers. Do not upload identification or financial records here. We will arrange a separate method if supporting documents are needed. If providing an address or contact detail could affect your safety, leave optional fields blank and request a safe follow-up.

Your responses will be used to review your request, contact you and prepare documents you authorize. Contact mirchi26inc@gmail.com about corrections or privacy questions, and the privacy notice for access, sharing and retention practices, including the retention agreement.

We are not attorneys. We do not provide legal advice or court representation. We are administrative transcription and document preparation specialists, helping you file your own documents accurately for a clean, predictable flat-fee.

Before proceeding with our client intake form, please check the box below to confirm that you have read and understood the information above.

We are committed to safeguarding your personal information in accordance with applicable Florida law, including the Florida Information Protection Act of 2014, section 501.171, Florida Statutes.

Review all statements about children, pregnancy, property, debts and support. Form 12.901(b)(3) includes a permanent alimony waiver by the petitioner. If a fact is uncertain or you do not understand the consequences, seek legal guidance before signing. Do not sign for your spouse.

This is an intake acknowledgement only, not a signature on the petition. The petitioner must review the final court documents personally.

Thank you. Your intake has been submitted for review. Submission does not mean your matter has been accepted, filed or approved, and it does not extend court deadlines. We will follow your safe contact instructions about missing information and next steps. Court documents require separate review and signatures.
